Training Consultation Form- fill this form out if you would like to set up Private Lessons, Ivy League,     Board-n-Train, Daytime Training, or Weekly Individual Training

Group Class Agreement Form- fill this form out if you would like to enroll in a group training class and do not want to sign-up using our Online registration

Liability Agreement Form for One-on-One Training/Private/Board-n-Train- this must be filled out and signed by ALL training clients that are booking one-on-one training or private lessons

Liability Agreement Form for all Group Classes- this must be filled out and signed by ALL training clients that are enrolling in a group class
